Purpose:
The purpose of the $1,000 Eden Prairie Health Challenge is to educate and
motivate people to make the lifestyle changes that will help
them live healthier lives while providing professional
support/ advice.
How It Works:
1. Schedule an initial 30 minute Health Challenge
Evaluation to document your current health baselines
including blood pressure, weight, skeletal muscle %, Body
Mass Index, Visceral fat %, resting heart rate, body fat %.
Cost is $20 and can be added on to a normal treatment visit
or scheduled separately.
2. Post your Monthly Health Challenge Calendar on your
refrigerator with the provided magnetic holder.
3. Track your daily activities/ points. If you have
questions or need support during the program, you can email
Dr. Kevin at any time at support@healthyforlifeclinic.com.
Program includes unlimited email support.
4. At the end of each month, bring your calendar into the
clinic or fax to us at 952.942.6295. Calendars must be
received by the 10th of each following month to earn points.
5. Monthly we'll award prizes to the top 3 points
finishers plus 2 random participants.
6. At the end of the year, the 5 participants with the
most points will share a $1,000 prize pool. 1st place =
$500, 2nd place = $300, 3rd place = $100, 4th place = $75,
5th place = $25
Patient Supplies:
Pedometer, 12 month calendar, fridge clip are provided at
the time of evaluation
Notes:
1. Program participants receive 25% off any vitamin
supplements purchased in the clinic.
2. Progress evaluations are available for $20 and
recommended every 1-3 months.
3. Replacement pedometers available for $5 each if lost.
4.
All participants also receive two (2) free Personal Training
sessions with a certified personal trainer at Fitness 19.

5.
Winner receives a free portrait sitting and 8"x10" framed
print with professional photographer Bob Dale of
Dale
Studios in Eden Prairie.
6. Top 5 points finishers receive a free healthy
cooking class
with Traditional
Cooking Arts
(up to $115 value)

Kid's
Version now available. Free for all children
of participating adults and a great way to start healthy
habits early in life. Great monthly prizes for kids!!
Kids simply need to come in to the clinic initially to
complete a 5 minute "information session" to review the
program with Dr. Kevin.
NOTE: Unless
specifically requested by parents, we will not track weight
and vitals for kids. The purpose is to simply allow
them to participate in a fun, healthy program with you and
earn prizes for making healthy choices. |
